BLOOD PRESSURE
Hypertension (high blood pressure) increases the risk of heart disease and stroke, the first and fifth leading causes of death in the United States.
MORTALITY
In 2021, Mississippi had 324 heart disease deaths per 100,000 population, compared to the national average of 174.
MORTALITY
The CDC has reported 30% of deaths in Mississippi were due to cardiovascular disease in 2018.
STROKE
80% of strokes can be prevented through lifestyle changes and medical interventions by addressing and managing controllable risk factors.
Healthy Heart Ambassador Program
The Heart Disease and Stroke program provides support, guidance and encouragement as participants monitor and record their blood pressure, giving them the tools to:
- Reduce high blood pressure
- Implement better blood pressure management
- Identify and control triggers that elevate blood pressure
- Develop healthier eating habits
This program is designed to help adults better manage high blood pressure through regular blood pressure self-monitoring and other heart-healthy activities.
Program Structure
- 4-month program
- 10-minute consultations with a program facilitator during drop-in office hours (at least two per month)
- Weekly check-ins from the program facilitator by phone, email or text
- Monthly nutrition education seminars

Joining the Heart Disease and Stroke Program is an investment in your health and a commitment to reducing your risk of heart attack or stroke. It promises consistent support to help participants develop self-monitoring habits to lower or better manage their blood pressure.
Participant Eligibility Requirements
- Be at least 18 years old or older
- Been diagnosed with high blood pressure and are on antihypertensive medication
- Not experienced a recent (within the last 12 months) cardiac event
- Not have atrial fibrillation or other arrhythmias
- Not have or are not at risk for lymphedema
